[ARCHIVED] SV Chamber accepting nomination forms for Phoenix Awards

In order to be eligible for nomination for any of the Phoenix Awards, an individual, business or
organization must have a physical location in the Shenango Valley, must do business in the
Shenango Valley or must be a member in good standing of the Shenango Valley Chamber of
Commerce.
You may nominate an individual/business/organization for more than one award category, but any
one of these can only receive one of the listed awards, with the exception of the Community
Pride/Beautification Award, which can be awarded to a winner of a different category. A nomination
package will include the following introductory information:


1. A single cover page with:
• The nominee’s full name, title, business and home addresses with telephone and fax
numbers
• The award for which the nomination is being made
• The nominator’s name, title, place of business, business address and telephone number
• A one paragraph description of the nominee’s business
2. A nomination letter (optional), not exceeding two pages, which would outline specific business
objectives, specific achievements, community support, etc.
3. A business profile, not exceeding one-page, describing specific type of business and the
business history of the individual, business or organization (may be included as part of #2).
4. Some of the awards may require verification of sales numbers, employee numbers and other
pertinent business and financial information. It would be preferable to include this information
with the nomination. Representatives from the Chamber will contact any companies that are
nominated if this information is not included and is needed.
5. Any other supporting documentation deemed significant by the nominator including news
clips, letters of recommendation and other evidence validating the nomination. Supporting
documentation must not exceed 5 pages. Videos will not be considered.

The Awards
New Business Startup Award
In recognition of sales and employee growth in a recently established business (18 – 36 months) in all
business sectors and with any number of employees or for any company or organization that acts as a
catalyst for new business startup.
Entrepreneur Award
In recognition of sales and employee growth in a recently established business (24 – 48 months) which
provided innovative and creative products or services. Includes all business sectors and any number of
employees.
Innovation Award
In recognition of efforts to increase sales and employee growth through diversification or product /
service development. Includes all business sectors and all levels of employment.
Growth Small Manufacturing
Recognizes sales and employee growth in the manufacturing field. Sales and employee growth must
be represented in the local market area. Growth comparisons are reserved for comparisons between
the previous calendar year and the year prior. (ie—2017 awards are 2016 vs. 2015) Small
manufacturing is reserved for for-profit companies whose employment levels are less than 50 FTE’s.
Growth Large Manufacturing
Recognizes sales and employee growth in the manufacturing field. Sales and employee growth must
be represented in the local market area. Growth comparisons are reserved for comparisons between
the previous calendar year and the year prior. (ie—2017 awards are 2016 vs. 2015) Large
manufacturing is reserved for for-profit companies whose employment levels exceed 50 FTE’s.
Growth Small Service
Recognizes sales and employee growth in the retail/service field. Sales and employee growth must be
represented in the local market area. Growth comparisons are reserved for comparisons between the
previous calendar year and the year prior. (ie—2017 awards are 2016 vs. 2015) Small Service is
reserved for for-profit or non-profit companies or organizations whose employment levels are less than
25 FTE’s.
Growth Large Service
Recognizes sales and employee growth in the retail/service field. Sales and employee growth must be
represented in the local market area. Growth comparisons are reserved for comparisons between the
previous calendar year and the year prior. (ie—2017 awards are 2016 vs. 2015) Large Service is
reserved for for-profit or non-profit companies or organizations whose employment levels exceed 25
FTE’s.
Non-Profit / Service Organization Award
Recognizes a Non-Profit organization that exemplifies dedication to the community and the clients it
serves. Its mission is consistent with improving the overall quality of the community and serves
clients in the Mercer County area.
Beautification Award
Recognizes a business or organization that contributes to the community pride of the Shenango Valley
through continued property maintenance, facade renovation, facility improvements or expansion, etc.
